The overflow occurs based on the queue timeout value. If the call is lost or the caller gets a busy tone even though you've correctly setup your huntgroup to overflow theres likely an incorrectly programmed timeout rule on the carrier settings that is less than your queue timeout value.
Change your queue timeout value to be a lesser value to resolve this or contact your carrier to increase the timeout on the line.
